If you download an application, the
application may either be faxed or mailed to us. See the
Contact Us page for mailing and faxing information. Regardless of whether
you fax or mail the application, you must mail the one-time application fee
of $25.00. You may send either a check or money order made payable to
Avenues to Medicine, Ltd. Co. or pay online via PayPal. We
cannot begin processing your application until the fee is received and
processed. |

Once we receive the application and
application fee, we will contact you for any information that is needed for
our completion of the paperwork.
Based on your medications and the requirements of the associated
pharmaceutical company, you will be required to provide a copy of any of the
following:
 | a Social Security Awards (SSA) Letter |
 | Social Security Benefits Statement |
 | W-2s |
 | and/or a copy of the first page of your last year's tax
return.
If you are required to supply a copy of your Social Security income
statement and you have misplaced it, you can call the Social Security
Administration at 1-800-772-1213 to request a copy. When you call you
should speak to an employee and request a "benefit verification letter"
for the current year Social Security earnings.

Once we have completed all the paperwork
required by the pharmaceutical companies, we will mail a Client Packet to
you with all the forms necessary to request your medications, along with a
checklist for you and your doctor to follow. |

You must follow your checklist/instructions
and sign the completed paperwork where indicated. |

Your instructions will advise you to take the
paperwork to the prescribing doctor. The Doctor's checklist/instructions
will advise the doctor of what is needed.
Once the paperwork is signed by you and completed by the prescribing doctor,
you must send the paperwork back to us for final processing.
Once we receive the forms, we will review them for
completeness, finalize them and send them to the pharmaceutical company. At
this point your medications have been ordered. We will notify you by mail
that the medications have been ordered and ask that you advise us when they
are received. We begin the monthly service fee billing approximately 4 weeks
from the time the medications are ordered |

The pharmaceutical company ships the
medication by either UPS or FedEx, on average, 2 to 5 weeks after they
receive the forms. However, in some cases it could take up to 10 weeks
to receive your medications. The medication is usually sent to the doctor
who prescribed the medication however, some pharmaceutical companies may
send the medication directly to you.
If the medications are sent to your doctor, the doctor should call you when
the medications arrive for you to pick them up. If you have not heard
from the doctor's office within 6 weeks from the time the medications are
ordered you should follow-up with the doctor or with us.
If the medications are being sent directly to you and you have not received
them within 6 weeks from the time the medications are ordered, you should
call us.
Note: If you need multiple medications, they may be
coming from different pharmaceutical companies. If the medications are
coming from multiple drug manufacturers they will probably not be sent at
the same time (i.e. some may arrive 2 weeks after they are ordered; others
may arrive 10 weeks after they are ordered). However, the important thing to
remember is that once you begin receiving your medications, you will
continue to receive them without interruption as long as you are in our
program. Because of the time it may take to receive your first free
medications, you want to be sure you have enough medications on hand to last
you until the medications arrive. |

Steps 3-6 must be completed periodically in
order to get your refills without interruption. You will also be required to
update your financial status each year. |
