Qualifications
To qualify for participation you must meet the
following guidelines:
| 1. |
Reside in the United States and have a valid Social
Security Number. |
| 2. |
Cannot have Medicaid Prescription Benefits. |
| 3. |
Cannot have Veterans (VA) Prescription Benefits. |
| 4. |
Cannot have Prescription Insurance (insurance co-pays are
not reimbursable) |
| 5. |
Can be any age. |
| 6. |
Can have a medicare supplement without prescription
benefits. |
| 7. |
Can have an AARP Discount Card or an HMO that pays only
for GENERIC medications. |
| 8. |
Must have a local prescribing Doctor. |
General Income Qualifications:
| 1. |
A single individual can earn no more than
$16,000/yr ($1,300/month) gross |
| 2. |
A married couple can earn no more than
$24,000/yr ($2,000/month) gross |
| 3. |
An additional $250 per month may be added to the above
amounts for each dependent. |
| 4. |
Income levels vary by pharmaceutical company and may be
higher or lower than the amounts stated above. Higher income levels
may be allowed for individuals with Cancer, HIV or MS. |

Note:

If you filed an income tax return last year, you
will be required to send a copy of the first page of your tax return with your
application. |

If you did not file an income tax return, you will
be required to send a copy of any 1099 statements sent to you at the beginning
of the year along with any other income documents such a pension statements,
annuity statements, or pay stubs. |

If you have NO income, the household income must
be reported before your application can be processed by Avenues to Medicine
Ltd. Co.. You must report any/all income sources in order for the
pharmaceutical companies to accept your application. Applications with $0
income will be rejected. |
